Once you find the defect what do you do?
Once I find a defect, I try to recreate the defect a couple of times to make sure that the defect is a real defect not a typing error or something else and it can be 'duplicated' or it reappears. I have used "Defect Tracking Tool" such as ClearQuest or TestDirector to log the defect. Later I submit this defect so that everyone can study it from the "shared drive".
